Our specially designed independent and care leaver accommodation welcome young people
who have previously been in our residential care and those who have been placed elsewhere.
As each of our supported living support services are individually packaged, costs can vary.
We can provide additional enhanced support as required.
The step down process between the residential and independence stages of the care continuum helps
a young person to familiarise themselves with self –reliance and allows them the opportunity to
request assistance should they need it.
Continuum’s Care Leaver workers make full use of existing networks within the community such as
Connexions, colleges, community police teams and local employers. During the young person’s stay our
staff work with them to ensure they find training courses or employment suitable to their aptitudes and interests.
In our shared homes each resident is provided with their own private areas and may share communal
areas such as lounges and kitchens. Our care teams are skilled in helping young people develop the skills to
build and sustain the support networks necessary to live independent lives. In this way we also ensure their
ability to make a positive contribution to society is maximised.
The Continuum Group also aims to support the Placements Northwest
Minimum Standards for Leaving Care Provision pilot to ensure delivery of quality services.

Case study – Zara’s story
Zara moved into a house in the community with a package delivered by the Family Support Services Team.
She found it difficult to manage being alone and the emotional impact of this resulted in risk taking behaviours.
A package was devised with Zara’s involvement and agreement in the Argyle Project, to meet all of her needs.
She thrived with the emotional support of the team who were on hand to assist and encourage her in all her social,
practical and independence skills.
Zara went on to complete the Prince’s Trust programme, gaining qualifications in food hygiene, manual handling
and first aid amongst others. Throughout her time in the Care Leavers Project, Zara gained confidence and life skills,
enabling her to realise her potential. After several months she was ready to move on and live independently
back in her home town. Family Support Service staff were able to guide and support Zara throughout this move.
