Wherever possible Continuum aim to keep young people out of residential care and to sustain their links with their home and family by targeting each individual’s needs in a planned manner.

Our Family Support Service has been established to complement our longer term work and to establish a solid foundation.

These services divided into three key areas each of which is subject to unique objective assessments and progress monitoring reviews.

Early Intervention and Outreach
Working with those young people who are at risk of accommodation and/or those who are experiencing difficulties at home which are affecting their education, progress and/or behaviours

Supervised and Supported Contact Facilitating more positive interaction between the young person, their family and the broader community.

Independence and Care Leavers Providing a supportive framework in the transition to self–sustained independence.
